Palliative Care Program

(718) 963-6394

Wyckoff Hospital has launched a new Palliative Care program.  Specialists in palliative care—physician, nurse and social worker—are available to help patients with any type of serious illness and their families.  The palliative care team can become involved at any time during the course of an illness.  Palliative care specialists treat pain and other symptoms, help manage other sources of distress, and assist with care plans after hospital discharge. When palliative care becomes involved, patients and their families are more likely to understand the illness, communicate their values and preferences for treatments, and take personal control of their future by preparing advance directives. If the illness is advanced and the patient and family might benefit from comprehensive care by a hospice program, palliative care specialists can help with referral and transitions in care.

Wyckoff Hospital has chosen MJHS Hospice and Palliative Care to assist in the development of the Palliative Care program and provide on-site hospice services.  MJHS Hospice and Palliative Care is a highly respected, not-for-profit organization currently serving patients and families throughout New York City and in Nassau County.
