H♥PE Home Visits Program for the Elderly

Goal: The program aims at keeping the elderly healthy, functional, and independent in our community as well as reducing ER Visits, Length of Stay and re-admissions. 

H♥PE is dedicated to providing comprehensive health care by nurse practitioners & M.D’s to homebound patients. While the program is mainly dedicated to our frail elders, we are also servicing younger patients who are fully or partially homebound and unable to access appropriate health care. Elderly patients mostly suffer from multiple illnesses, functional decline, and dementia, while some younger patients may suffer morbid obesity, cerebral palsy, multiple sclerosis etc. By providing home visits on a regular basis, patients are closely monitored for their functional decline, medication compliance, and provide preventative care.

Acute Geriatric Unit

As of 2011 we have also established 10 dedicated geriatric beds on the 5th floor for elders. An Acute Geriatric Unit is dedicated to providing both standard medical services and advanced geriatric interventions. We are geared towards multidisciplinary approach for preventing functional decline, sentinel events (falls, decubitus ulcers), decreased length of stay, readmissions leading to improved transition of care as well as patient/family satisfaction.

Palliative Care Services

At Wyckoff Heights Medical Center, we have a dedicated palliative care team to address these sensitive issues. The service provides the medical expertise on helping patients and families with understanding the disease and prognosis. 

Palliative care is an approach that improves the quality of life of patients and their families facing problems associated with life-threatening illness, through the prevention and relief of suffering by means of early identification and impeccable assessment and treatment of pain and other problems, physical, psychosocial and spiritual. 

Palliative care Team:

  • provides relief from pain and other distressing symptoms;
  • affirms life and regards dying as a normal process;
  • intends neither to hasten or postpone death;
  • integrates the psychological and spiritual aspects of patient care;
  • offers a support system to help patients live as actively as possible until death;
  • offers a support system to help the family cope during the patients illness and in their own bereavement;
  • uses a team approach to address the needs of patients and their families, including bereavement counseling, if indicated;
  • will enhance quality of life, and may also positively influence the course of illness;
  • is applicable early in the course of illness, in conjunction with other therapies that are intended to prolong life, such as chemotherapy or radiation therapy, and includes those investigations needed to better understand and manage distressing clinical complications.


Apart from advanced cancers, there are various other conditions who will benefit from palliative care. These are, advanced COPD (emphysema), Congestive heart failure, HIV/ AIDS, Dementia, any end stage disease etc. The patients and families are counseled and educated on advance directives, and guided through their time of grievance. 

Geriatric Clinic

Geriatric clinic is available for our functional seniors in the community. It aims at managing co-morbid illnesses, maintaining functional status, memory evaluation, and exploring any psychosocial issues.
