<!--[if !supportLists]-->·         <!--[endif]-->Receive and analyze all incident reports to identify possible reportable events, potentially compensable events and unusual or developing trends. Prepare various incident summary reports for hospital leadership; provide all required documentation for investigations, activities and resolutions related to the reported incident. Maintain incident report archive.

<!--[if !supportLists]-->·         <!--[endif]-->Investigate and track clinical risk issues as may be identified and reported by other departments.

<!--[if !supportLists]-->·         <!--[endif]-->Identify and report adverse events that meet NYPORTS reporting criteria.

<!--[if !supportLists]-->·         <!--[endif]-->Assess need for focused review by departments, quality of care reviews and peer reviews. Coordinate Comprehensive Systems Analyses (Root Cause Analyses) for adverse events with Chief Medical and Nursing Officers and with relevant and appropriate clinical and administrative staff and to develop appropriate corrective action.

<!--[if !supportLists]-->·         <!--[endif]-->Act as liaison with the NYSDOH/NYPORTS coordinator.

<!--[if !supportLists]-->·         <!--[endif]-->Review and summarize patient charts relative to clinical incidents and malpractice cases as necessary.

<!--[if !supportLists]-->·         <!--[endif]-->Assist medical malpractice defense counsel with information requests, identifying witnesses and gathering documents. Review malpractice case reports to identify opportunities to assist defense with clinical or organizational information and analysis. Attend malpractice claims conferences.

<!--[if !supportLists]-->·         <!--[endif]-->In consultation with in-house counsel, refer potential malpractice claims to third party claims administrator for opening unasserted claims files.

<!--[if !supportLists]-->·         <!--[endif]-->Assist in-house counsel with investigating general liability and property claims; liaison between hospital personnel and insurance carriers/outside counsel as necessary for investigation and discovery; respond to insurer/counsel requests for information.

<!--[if !supportLists]-->·         <!--[endif]-->Advise clinical staff on issues involving the informed consent process (capacity, identification of surrogate decision maker, proxy, refusal, etc.).

<!--[if !supportLists]-->·         <!--[endif]-->Attend and participate in meetings of various hospital committees: Quality Management and Patient Safety, Environment of Care, Ethics, Forms, Claims and O.R.

<!--[if !supportLists]-->·         <!--[endif]-->Participate in weekly Environment of Care tours with other members of EOC Committee

<!--[if !supportLists]-->·         <!--[endif]-->Develop new hospital policies to address new risk management issues. Review and revise, as necessary or required, current risk management related hospital policies.

<!--[if !supportLists]-->·         <!--[endif]-->Oversee and direct Patient Advocate and Limited English Proficiency programs and personnel.

<!--[if !supportLists]-->·         <!--[endif]-->Accept legal service of process as necessary. Liaison with law enforcement and government agencies regarding medical record and witness subpoenas, bedside arraignments, etc.

<!--[if !supportLists]-->·         <!--[endif]-->Staff education as requested; prepare training videos for new employee orientation and annual employee in-service.

Required: BSN, 10 years’ experience in hospital risk/quality management/administration.
